Kouichi is interesting here because clearly he doesn't actually mind being a girl sometimes, even enjoying being cute and looking out for outfits he could wear when TS'd but clearly the idea that he is very very likely to stabilize as a girl due to how late the TS'ing started for him is inducing some distress. That alongside the fact that everyone else just keeps taking advantage of her 10 year old form for dress-up.

It really feels even if unintentionally like Kouichi would have a much easier time exploring the idea of being a girl if eveyone just chilled and give him the time to figure things out for himself. Instead he's clinging to experimental drugs that will absolutely backfire. It'll be interesting to see where things end because while this manga has risen above just being a GB harem, it's still not at all thinking about things in terms of real life trans people. I'm sure it'll be entertaining nonetheless.
