It'll be explained later, though the implications are laid out in parts the story already skipped. Very small spoiler as to why follows:

Basically the ducal house she is in is very old fashioned and only allows male heirs. Her father never had a son, so forces Karen to act as a boy. Everyone knows Karen is female, but. . .

Honestly, the story in the manga skipped a few things for setup and changed a few small details but we are still in chapter 1 of 3 for volume 1. Karen's stuff might be more expanded on when we get to her focus point.
