Just a shot in the wild that's more likely BS than not-- but I heard that when surgeons perform brain surgery they don't give patients complete anaesthesia. Could it be that he'll do the same, and periodically try to talk with him while he's operating, to make sure he's not screwing up? I'm 90% sure this ain't it but I do wonder what kinda solution he thought up that the big hospital hasn't thought up of

@Lithe Dr. Koto literally said "you plan to unilaterally say only what you wanted to say and die?"

I couldn't really think of a natural way of saying it in English. I didn't feel like unilaterally would sound like a natural way to say something in dialogue like this.

Mom telling her son that it was more than enough for her that he was around--that hit me right in the kokoro.

My mom died from stroke. I remember during my mom's last few hours in the ward, I was told that if she does wake up, then she'd probably lose mobility etc. And I was thinking, "I don't care, as long as she's still alive. I'll devote even my whole life caring for her."

It's been 6 years, but I still do miss that woman so much. :(

Hoshino's "brilliant idea" that Dr. Koto gets so excited about? That's literally how they do brain surgery on tumors near critical sites.

I feel the mangaka dropped the ball on their research for this episode.

@freakofnature Please take into account that this volume was published in 2002, and I can't find an exact date, but it looks like that awake craniotomies started getting introduced in Japan in the 90s, maybe late 80s. (Don't quote me on this though).

That's a good point, and I had forgotten to account for the story being of relatively older vintage. Even so, I can't imagine a highly skilled surgeon from around 2000 being unaware of the existence of this method. Of course, the mangaka's level of knowledge is a different matter.

or, how widespread the procedure was.

it's like how if someone were to make a manga today about smoking, vaping would be introduced as a new thing, whereas many years in the future, it may just replace traditional cigarettes.

Kinda missing the point a little, there. It's been established that Dr. Koto is highly skilled and well-informed about his field. Don't you think he ought reasonably to be expected to be aware of the technique?

and what exactly is his field? we've seen him perform all kinds of operations.
because if it's general surgery, then it's understandable if he isn't aware of what may be brand new techniques in a specific specialization.

then again it's more likely down to the author.

I mean, he saved the guy from metastasized cancer, a brain tumor should just be a walk in the park...
