He's been gradually becoming more girl and less boy all through the manga. He even went to school dressed in girl's clothing (complete with bra & panties) back in chapter 17. It was an accident, but it really showed how he's thinking of himself more as a girl than a boy.

Genderbender manga gives us some of the weirdest sentences:
