I think you missed the point. Nervousness from being naked was not the reason she really blacked out.
Due to her past experiences where her partner would just dump her after a few times, she got anxious about it happening again with Kiyomi.

Well although it is the usual delay troupe. Honestly this time at least the reason is not so half assed as in other stories and is in a way legit.

I mean it might be a stupid logic, but then again, since when emotions were logical? Especially when you are young and stupid.

I do agree that the manga went with a cliche route. I guess the author decided to develop their feeling a bit more before doing the deed.
I am just glad that the reason was at least somewhat understandable and relatable, instead of a case where a healthy teenage boy develops a shy erectile disfunction because reasons.

I still think that the whole manga is crippled by the 13th page (+/-1) of the first chapter. I still can't buy into her "true character" because of that "hentai slut" expression. It's really an art in on itself, that the author portrayed her as a sex-driven person for like 3 pages, and it managed to undermine an almost 500-page long story after that. Everything after that works - maybe not as well as "It's Not Meguro-san's First Time", or "Experienced You and Inexperienced Me", but it's still pretty good. But I still feel a disconnect between the girl in the first chapter and the rest of them. Maybe I'm just weird.

In the end, Nanjou-san still has come to the conclusion:

The coughing and blacking out due to anxiety was actually really interesting. We were in her head the whole chapter, and there were a lot of hints that it was getting to her. She constantly talked about how nervous she was, and once she started having the acute symptoms of anxiety she admitted she was afraid.
I like that we see why, too. That she had it, somewhere in the back of her mind, that when they finally had sex this was all going to be over. The thought physically scared her, to the point that she had symptoms. It's a really interesting character moment. We've heard her be confused over how excited and nervous she's been before, and could guess at why she felt that way, but now its established. She likes the feelings she's having, she wants to connect sexually, but she's scared that once she does she'll lose this relationship like she always has in the past.

It's as sweet as the rest of the manga has been, too, with the chapter ending with them hugging each other in bed after having a good time just hanging out and enjoying each others company. Honestly, this thing's adorable.

Well even if you look at her behavior, the more their relationship progressed, the less she was pushy about sex. Like yeah she is still throwing lewd remarks, but those got toned down to an extent and her reactions were also more contious.

People are still hung up on the 1st chapter of this series like character development isn't allowed or people (characters) aren't allowed to have any depth or progress past their first impressions.
Honestly I think that people just tend to not dig deeply into stuff they read and just wanted a light read of romance with suggestive scenes like in other most mangas in this genre.

Even this story was instantly rated on mangadex with quite a low score just because the female lead in the start was a thod, even though the story was hinting into a fact that she will be developing as a character into a different mindset.
