I don't think she's dead, the wedding photo was broken and forgotten under the bed. You have to be a particular piece of shit to leave such an important memento of a deceased loved one in such state, and Manager-san doesn't give this vibe. I stand by divorce -> porn.

It's not the reason he got divorced, we still don't know anything about the wife. Anyway he got divorced 2 years ago, and while walking in the rain like a broken man he walked by the adult store. There he saw Yuri's poster and noticed she looks like his ex wife.

Most likely "porn addiction caused by a broken marriage" or "dead wife" because "divorce caused by porn addiction" would honestly be something to much for this comedic ecchi manga and would really change the vibes of this manga to something more dark and "dirty".
