What was the reason for the scar in the novel?

Is it me or this romance came out of nowhere? Like I didn't see it coming at all. The entire time it was shown that she is only interested in Makoto but now I am supposed to belive she actually loves someone else?

Maybe I missed it somwhere but was it explained whether the exp gets depleted from the guys or does it regenerate after some time.
