Looking at the clothing and architecture, this setting isn't feudal. It looks to be around 18th century, in the middle of the industrial revolution if not post industrial. Most countries would have constitutions and liberal laws at this point even if the aristocrats still held...

Otome games don't even have villainess characters for the same reason that galge don't have competing love interests. These "otome game" manga are actually based off old school shoujo manga.

Saber's name is supposed to be Artoria, female version of Artorius, which is the Latin version of Arthur. 'Altria' only exists because the official translators are ignorant.
@Thelania Depends on how much fat she's actually got stored, really. There was once an obese man who ate nothing but water and vitamins for an entire year and in the end he just about reached normal weight.
