We know Lulu is the final boss since he agrees her to be the last in order to free the house. While he is going down the different levels (siblings). My guess is at the end he will need to decide a sacrifice where either he ends Lulu freeing the house for the couple but traps himself or where he leaves Lulu alive and leaves probably to learn a horrific fate has befallen his mother when he stopped being able to afford treatment
