As far as Manga goes, the word "Hag" has always been used in a negative connotation.

Aka., used on any women who is past their prime (aka., no chance of ever finding a partner) or cranky or ugly looking.

Also, a person who likes Grannies wouldn't personally call them Hag. Even if the usage of the word might have changed because of modern slang that doesn't mean it's past usage meaning has been forgotten.
