This comment was meant for chapter 40, but chapter 41 was release as I was writing it, so I'll put it here instead:
As I've seen some comments here that appear to have missed some details. I'll try to resume to help in keep up with the Author's Mystery Box, and even give who I predict who's the actual killer
In regards to "Fake" Miyu, from what has been showed and mentioned, we can assess that she's not the real one, and is only impersonating the "real" Miyu, especially when on chapter 17, the start of Volume 3, the colored first page shows the two walking together, and it's not implied to be "Fake" Miyu reminiscing about the past (at least in my view)
As for the "Fake" Miyu appearance, it's mentioned that although she looks very similar to the real one, there are some differences here and there, such as the eyes, both in shape as mentioned by Hongo, but also in color, as "Fake" Miyu's eye are more pink, while the eyes of the "Real" Miyu were more orange-ish (as per the colored page of chapter 17). And also from a lack of a mole on her stomach, which is mentioned in chapter 2 that the real one had, but in chapter 34, where the "fake" Miyu fully undresses herself there's no mole on the stomach.
While her birthday, being the same as the "real" Miyu, it's said by the "fake" Miyu, it's mentioned by herself in chapter 13, that she was born in a toilet and that her mother didn't even know what date it was, so she just picked an easy to remember date, which in this case was the 3rd of March (3/3).

Now in regards to the suspects of being the killer
I don't think it's Highway Suguru, because also in chapter 34, it's show that he was searching news about "real" Miyu's death while he was by himself in a train, and that he sounded rather surprised in finding out that she was actually dead, and that the pool of blood belonged to the "real" Miyu.
As for Yuushin, because so far, the story does follow a third view mostly centered on his perspective, there's a possibility that it could be him, but I also don't think it's him as of our current evidence, due to chapter 1 showing that Miyu had messaged him at 17:02, and he messaged her back from the hospital (Though we don't know at what time he answered) on the day of the festival, which if I had to guess, could have put his injury happening at 16:00 or a bit later.
And this is partially supported by the fact that Suguru had been following him during the day of the festival and was the one who called the ambulance.
(as for why Yuushin fell, it's said by himself that on the day of the festival, he encountered Murai, who act as if he was going to hit him, Yuushin overreacted and ended up falling the stair. Though this one is an unreliable one as well, as it's from his perspective).
Now, because of this chapter, is Hongo Haruto or his brother or both, the killer? I will be honest, I also doubt it. Why? Timing, simply put the story that Arisugawa Hina told, just don't make sense, because if she saw Haruto walking out of an alley without his cap first and then heard "Real" Miyu hitting the ground, there's simply no way that this could've happened, as Haruto would need to have super speed to somehow push Miyu out of the building and then get back to the street, as it would literally just take no more than a few seconds for Miyu to fall from 18 meters.
But also, from what we saw between the Hongo brothers and their relationship, it's really hard to say that the two would just cooperate to just kill Miyu and them go back to the older brother being an asshole to the younger one.
Of course, could the two have left the house with the same clothes on the day of the festival, and Arisugawa simply mistook the two for the other? Possibly, as we only have Haruto's side of the story of his brother's NEET life and how long he's been like that, and to a certain point, Haruto does have a reason to kill Miyu as he likes Yuushin, so he might've committed a homicide of passion, but by his reaction in the last chapter of finding out that the current Miyu is a fake one, it doesn't appear have been him, unless he's a damn good actor.
And the older brother, doesn't appear that he left the house before his cover was blow, however this is just an assumption of mine of the character, nor does it looks like he would have had any reason to want to kill Miyu or even bother himself to kill someone.

Now who do I think is the killer
This will sound like a curveball, but it does make sense some sense, the killer of "Real" Miyu… Was "Fake" Miyu's father.
This assumption of mine is based on certain points:
1. In chapter 6, in the first page we see the scene of "Fake" Miyu looking at a burned apartment in the past, possibly her home, which on the context it was being told, it does appear to imply that it was caused by someone.
2. In chapter 12, page 20, we see what could possibly be her parents fighting, or maybe her mother making out with a lover, or something else, and she's sitting outside of what appears to be the apartment showed in chapter 6.
3. Chapter 13, "Fake" Miyu was born in a toilet, which could imply that she was born from some affair or simply that she was ignored by both of her parents.
4. Chapter 7. The letter "Fake" Miyu receive, she says it was delivered to her by mistake as the two are name "Fukagawa Miyu" and lived near each other
5. We don't know what happened to "Fake" Miyu's parents or where they live/lived
6. "Fake" Miyu looks like "Real" Miyu, but the opposite is also true, "Real" Miyu looks like "Fake" Miyu
And since we haven't fully seen or heard from "Fake" Miyu how her family was, but we've been given some details to assess that it was a rather bad family, and with most likely a lot of issues. From contextual evidence, we can assume that someone burned down the apartment she used to live in, someone knew her name and address (or general address), and this someone also knows who the "fake" Miyu really is.
Taking all of this, there's a good chance that "Fake" Miyu's father is the killer, since we never heard of her parents, he might live in the Gunma prefecture, he could have the reasons for wanting to kill her.
So what could have happened is that the sent letter was meant for the "fake" Miyu, but because the "real" Miyu also lived close by, she thought it was addressed to her instead, and "real" Miyu was going to the festival to meet with Yuushin, and because her appearance is almost exactly the same as the "fake" Miyu, the father of the fake could have mistaken her, and thus ended up killing the "real" Miyu.
What about the sports cap though? In my opinion, very likely a red herring. As a simple sports cap can be bought anywhere by anyone, so unless there's any hair of its owner still stuck on it, and they decide to make a DNA test with it to find out who owned it… I don't feel like that it'll belong to Haruto or his brother.

If this is true, then rip the author, as the audience definitely won't like it
Since Haruto and his brother are twins, the timing of one leaving the scene before Miyu's death doesn't preclude one of them from being the killer. It could be that the one who went to the alley with the hat is a different twin than the one she saw leave, or maybe Miyu was supposed to meet with the one that had the hat and the other took it from him. I don't think either brother is the real culprit, but Tsukito probably knows more than he initially let on, and one or both of them might have been involved in some way.
The theory about fake Miyu's father is interesting. I think her past definitely will have a role to play in the near future (unless Axe-kun says otherwise), but I don't think her family would be involved with real Miyu. If the killer turns out to be some random character we haven't seen yet, I think a lot of people would find it to be an unsatisfying resolution to the mystery. Ironically, I think the killer being real Miyu's father would be a more likely twist. In the end though, I would guess the killer will turn out to be either the last classmate we have yet to meet, or a character that's already been introduced.

ok, that is actually a cool plot twist, but I also doubt it to be someone who has not been involved in story and set-up (old classmate) so far, it would be a very dissatisfaction to the reader that everything has been a waste of time
but the idea of the culprit being related to the fake Miyu, or more correctly, the death of real Miyu being related to fake Miyu could very be the endgame (consider how she is so determine to find the truth), like culprit mistook them, accidentally approached the fake Miyu (and she had no memory of this) before meeting the real Miyu, didn't know they were different people, something happened and cause the accident
heck, I wouldn't surprise it turns out to be just an accident after all

Yes, I do acknowledge that the two could be in the same area, and that Arisugawa could have mistook the two on the day of the festival. But as I also mentioned, considering what we have available about Tsukito, it's hard to think that he would've left his house before the events of Volume 3, as he's a NEET, appears to want to be mostly out of trouble, and doesn't appear to have a motivation for him to kill the "Real" Miyu
Of course, him fainting after seeing "Fake" Miyu in chapter 21, could be interpret as it being from him seeing someone who was supposed to be dead standing in front of him, or just him being weak to women, as supposed by Haruto in the same chapter.
And to an extent, Tsukito does appear to not have been fully aware of "Real" Miyu's death, until Arisugawa revealed it in chapter 39, but because he is mentioned as being a genius, he was probably able to connect some dots here and there to guess what happened to "Real" Miyu, especially when what he knew in regards about the "real" Miyu being from what Haruto talked to him, before Tsukito became a NEET.
Now, yes. He does appear to be aware of something regarding "Real" Miyu's death, and we need to see if that will be useful information or a Red herring.
As for the "Real" Miyu's father, being the killer, I don't think that's possible considering the lengths he has currently gone in investigating her death. Especially when in chapter 7, "Fake" Miyu says that "Real" Miyu's parents provided the letter to the police as evidence that she was murdered, but the police still refused to investigate, as the Japanese police tend to do, he could've just stopped there if he was the killer, instead of becoming a detective himself
That's pretty much my assumption of why I think the killer could be "Fake" Miyu's father. (Though, it could also have been "Fake" Miyu's mother, or even someone close to one of her parents)
Whether this person planned to kill "Fake" Miyu or not, the fact that "Real" Miyu looks like "Fake" Miyu, they could've confused her for the fake one on the day of the festival, and ended up killing the real one. Be it because they were planning to kill "Fake" Miyu or some confusion that happened between them and the "Real" Miyu, and accidentally ended up pushing the "real" Miyu to her death

As for the reason I didn't assume it was one of the old classmates, is because of the remaining primary suspects, as only Arisugawa Hina and Hongo Haruto have any possible motivations to have killed the "real" Miyu. While the other classmates from he class reunion, appear only once and aside from a few who are show photos of their middle school selfs, none get any sort of extra attention to make them a suspect.
As for other previously showed characters, again, either there's a lack of a proper motivation or screen time to make them a suspect.
As such this only leaves us with the following suspects, and also assumes none of them are really good at acting and faking their reactions
Arisugawa Hina
She has the motivation to kill Miyu as she was close to Haruto, she has the resources to know where Miyu moved to, and she's delusional about being the fated partner of Haruto.
The main problem here is mainly that, we don't know if she's really telling the truth, especially as her story does make Haruto or Tsukito into the likely culprits, even though she says that she'll protect Haruto.
And if she's lying to protect herself, that means that from Haruto's story about her being delusional since she was 3, means that she's been keeping that act for 15 years at minimum, which doesn't make a whole lot of sense in this case, since that would mean she is not delusional.
As such, considering everything so far, it can be assumed she's being honest to show that she's innocent and that she's really trying to protect Haruto by telling the truth of what she saw.
Hongo Haruto
Similarly to Arisugawa, he would have some motivation to kill Miyu, as he has feelings for Yuushin, and his reaction in seeing "Fake" Miyu for the first time, did look suspicious.
Right now, as where things are pointing, he is the most suspicious, but as "Fake" Miyu assessed in regards to his personality and what happened in Volume 3, he hardly appears to be someone who would try to harm other, and instead he tried to let out his frustration by punching the wall in his room
Hongo Tsukito
Although suspicious, he simply lacks a motivation to kill Miyu, and according to what we know about him, by the time of Miyu's death he had been a NEET for 2 years, and he had to act like a madman so that his parents wouldn't just go back into pressuring him to study and have perfect grades.
With that in consideration, it's hard to believe that he would risk leaving his house during the festival, and potentially blow his cover back then.
As for Yuushin
He simply has too much of a solid alibi to prove where he was and when by the time Miyu was killed, backed by both Murai and Suguru, and from chapter 1 we can also say that by 17:02, Miyu was still alive due to her message, and he was already at the hospital with a cast on his broken leg by that point.
"Real" Miyu's father
As mentioned before, his current actions simply don't make sense if he's the killer, as he could've just kept quiet as the police refused to investigate the death of Miyu
My assumption of it being "Fake" Miyu's father
Although we never heard of "Fake" Miyu's parents to fully know how they were like, of what little we know, we can assume that they were bad parents.
So they could either have planned to kill her but mistook her for the "Real" Miyu on the day of the festival. Or they had some other plan with "Fake" Miyu, but when they confused "Real" Miyu for her, this led to some arguing or fight, which led to an accidental death of Miyu
And a very curveball suspect, "Fake" Miyu herself
Why? Because so far, most of the evidences that we have, came mostly from her, in other words, she for the most part, has the control of the narrative, she looks almost like the "Real" Miyu, and this whole investigation could be just a way of keeping any suspicions away from her.
Though this has its problems, as it's pointed out multiple times that she liked, or outright loved, "Real" Miyu and wanted to be with her as much as possible, so it's had to imagine that she would just kill Miyu to replace her.
She doesn't look like she would have the money or even leeway to be able of going to Tokyo, buying stamps for the letter, and then going back to Gunma Prefecture, to write the letter from a computer, and then handing it to "Real" Miyu. And then do the same commuting again, on the day of the festival.
And she would also have needed to scout for a place to kill Miyu before writing the letter, which again it doesn't appear she would have the time or money to do so
Or for an even more curveball, Murder in the Express Orient
As in, there's no one single culprit, and instead everyone had a part in the death of "Real" Miyu in some way or another

So what do we have that we can rely with some certainty?
1. A letter addressed to a Fukagawa Miyu was sent asking her to come to a certain building in Asaka at 17:00 on the day of the festival
2. By 17:02 of the festival day, the "Real" Miyu's message to Yuushin said that she was almost arriving and was going to a station
3. By 17:10, she was killed after falling from a building.
4. In chapter 34, on the small bits we can read of the news that Suguru is reading about Miyu's death, it mentions that the owner of the building likely says that the girl entered the building without permission.
So this leaves us with the following according to our current solid evidences, at the time, someone knows that a Fukagawa Miyu lives in Gunma prefecture at a certain street address, this someone sent her a printed letter asking her to come to a certain building rooftop in Asaka during the festival day at 17:00. And depending on what type of building it was, Miyu could only have entered if someone opened an entrance for her and also opened the door to the rooftop for her, if it was locked. And then, pushed Miyu to her death.
So this killer, needs at minimum be someone who lives or works in Asaka, Tokyo; knows that a Fukagawa Miyu lives in Gunma prefecture and her street address; And has access to the Asabana 3rd Studio building and its rooftop.

I see, shit is about to go down and everyone is theorizing. I think a re-read could make things even clearer, I also don't think that if they find the suspect everything just ends. This author is good at dealing with the aftermath. Thanks for the TL

Just from the title alone, I still hold Yuushin suspicious just by the hope that it's not someone suddenly appear as a culprit without any appearance before at all.
