Looks like they’re subverting the porn trope of genderbent guy getting feminized by his body by giving Ishua legitimate body dysmorphia. It also helps in emphasizing his previously expressed inadequacy around not being able to wield his sword. I’m thinking the reason he like the boot camp instructor talk is probably because of how overly-masculinizing it is rather than the fetish thing (as I saw suggested by another comment).

It really is incredible how the author manages to work the sex jokes into the plot without it feeling totally unnatural.
