I wonder if this manga is going to treat his DID seriously (which is severely debilitating IRL if you ignore tiktok fakers), or just use it as an excuse for the dude to switch between ignoring and lovebombing her without coming off as a manipulative douchebag.

Yeah, it's a little too cutesy, like they wanted the 'shoujo-jerk' trope but only half the time. Someone with his condition would require a lot of care and attention, and he just got dumped in the countryside like an abandoned puppy.
I smell a hint of "Don't go up those stairs. The mountain God will play tricks on you, like giving you a diagnosable mental health condition!"
That one is realistic considering that both of his parents are alcoholics.
Less realistic is how he's able to attend school without the teachers mentioning his condition (or even knowing about it?), I mean sure it's Japan where disabilities are shamefur dispray, but with his massive lapses of memory he's gotta need some special accommodations.

Am I the only one that thinks it's not DID but actually Riichi from the future constantly swapping places?
1) The manga title is Start Over, like regression.
2) The constant flashback to a girl that got into a traffic accident is probably Akira in the future. The uniform is the same.
3) Akira noted that Riichi looks like he's going to cry when he sees her in Chapter 1 and 2, which would makes sense if Akira is dead in the future (the cover image says it will be a bittersweet romance).
4) If it's really a different personality, Riichi has only met Akira a few times. It seems unlikely he will be willing to hold hands with Akira. But it makes sense if Riichi is from the future and they were dating.
5) Riichi knows her brother's name when Akira never told him.
