A lot of that depends on the couple. When my mother's parents went out to eat, it was expected that my grandfather would speak for my grandmother - that was just the culture they were raised in. She didn't talk directly to the waiter/server. Yeah, it's weird to me (and smacks of chauvinism, as well as other things), but I can understand why someone who was raised that way would be uncomfortable doing it 'normally'.
