IIRC,
When Tenshi (Kanade) stab MC (Yuzuru), she realize that MC doesn't have a heart.
That's why the sadness in her eyes (cuz Kanade has the heart that MC donates in the real world when they're alive).
And also that her wish will finally be granted in the end with MC.
