So yeah I haven't read this manga for a while can anyone explain to me what happen? Last time I saw her still young then she become old after she tried to find her car, is that her being young just seiichi illusion?

Seiichi is making the same faces Seiko used to make, just a mix of pure apathy and a tinge of curiosity. The whole world could end and Seiichi would probably still take care of Seiko at this point. Seiichi is ultimately the winner, ya he's kinda fucked mentally, but unlike Seiko who just abandoned the relationship, he's gonna see it to the end. She may have started this but she won't end it.

He's given up on everything outside watching his mom die, I'm concerned he'll try and kill himself once it happens

So the last pages are Seiko's 'daydream', right?
I think it will be the last conversation before she dies. I hope she at least feels sorry for what she did to Seiichi.

She's dying and he's taking care of her, promising to watch her to the end. As usual, his emotions while doing this are muddled and unclear—is it relief? Depression? Or acceptance?

The daydream of the both of them drinking iced tea could be her dying thoughts, or Seiichi's imagination of her dying thoughts, or an unrelated daydream. It remains to be seen.

Since Seiichi realized that the roles in their relationship has been switched, he became the carer and his mom became the infant. So I assume that his motive is to be just acting different than his mother in the same situation, since "he is his own person" and all.

...And that morbid curiosity of how the giver and the destroyer of his life will fall apart in front of his very eyes is a strong reason too. The thing is, when Seiichi had a conversation with his mom in this manga, his condescending inner voice always got the best of him. So him being a content, wise dude for last dozen of chapters might be coming to an end. He might start to question himself and beat himself over yet once again.

I don't think he is taking care of her out of love. It seems to me he is doing it more for some odd satisfaction in seeing her becoming undone than anything else. If i'm correct, i understand that she screwed his life on several occasions, but this will not do anyone any good. It would be best to just move on.
