This is supposed to be an award winning manga... Would it have hurt to make it a bit more realistic from the beginning? They could have saved insane operations (with no facilities) for a special arc, like when a typhoon isolates the island and all access to the mainland is out of the question (then they wouldn't even need to pretend helicopters don't exist).

@Kaarme I don't see there is anything unrealistic. And there are lot more diseases that are even more dangerous that need multiple doctors and nurses.

yeah, maybe not the most realistic doctor. but one thing that is super realistic is old people refusing treatment. my granny hid her cancer until it metastasized (gramps died of stomach cancer years before).
