On one hand, his sister fell to the female biological misevolution of "I can fix him"-delusion. Silly girl.
On the other hand that guy was so fucked up in his mind, that he didn't notice that the life of his dreams was in his grasp.
As others said, Raoul couldn't torture him more if he tried.
If I understand that correctly he won't even die of anything besides maybe old age. Stuck in an infinte loop realizing what he threw away, what he did and being reminded of the one women geniunely loving him, whom he betrayed again and again. Maybe, in the end, both his mind and soul might be completely eroded, reduced to oblivion.
And he deserves every second of it, since in the end:
Events don't change a character, they merely reveal it.
