Nah, he's not really unscathed. He must spend the rest of his life in eternal regret. Living the rest of his life, tormented, then dying, going to Hell, and being tormented further. After all, we know Hell exists in this world.

But also, I think Raul didn't go super brutal on him, not because Raul didn't want to, but to respect his sister's wishes to be kind to him. That's the most kindness he could muster up, though. Letting him starve to death, apologizing fruitlessly to the one person who actually loved him.

Guilt hurts.

On the other hand that guy was so fucked up in his mind, that he didn't notice that the life of his dreams was in his grasp.

If I understand that correctly he won't even die of anything besides maybe old age. Stuck in an infinte loop realizing what he threw away, what he did and being reminded of the one women geniunely loving him, whom he betrayed again and again. Maybe, in the end, both his mind and soul might be completely eroded, reduced to oblivion.

And he deserves every second of it, since in the end:
Events don't change a character, they merely reveal it.
