No, the old man escaped because the MC felt the need to talk shit and then start to threaten the geezer with all manner of chuuni fates-worse-than-death. It's a mistake made by both amateurs due to hesitation and people who can't keep their head cool. Canaan only got the chance to interfere because Tsukuru was taking all the time in the world to kill the man. What's the point of torturing him or any of that shit? Rather than talk shyte and paying attention to Canaan, he could've just drive more pain into the old man, torturing him & healing him so that he couldn't stand anymore, to make him suffer that he would beg for death himself. Only then should MC begin to talk shyte while driving more pain to instill more suffering to the old man so that he would break mentally and physically.
