Is it confirmed she’s in a coma from the truck accident?

No, not at all. Nothing's confirmed. That is what Koharu herself thinks is happening, or at least used to think in the earlier chapters.

In those early chapters, Koharu also worried about the implications of a 35-year-old lady interacting with high school boys on a daily basis and briefly imagined herself getting arrested, and we saw recently she hasn't let go of that worry at all, so chances are she's also still worried about whether what she's experiencing is all a dream she's going to wake up from. For better or for worse, we should get an answer eventually.

That being said, in the most recent chapters, Koharu did move on to other theories that involve the supernatural. (Replacing her parallel dimension self and such.) It's all still very up in the air.

I personally hope we get a time skip to her as an adult again after she finishes her school years. Y’know, she just genuinely grows up again. Is thay cliché? Maybe, but it's certainly a do-over.
