If you care about history, "Boku no Shotaiken" 1975 is the oldest gender switch manga I know (well also Princess Knight but it isn't really about the physical change but just mental). And well, piss plots are already a decade older in gag manga, Susumu being part of Nagai's Dynamic Pro so it is not unexpected. That reminds me, Nagai Go did a gender switch drug plot in Kikkai-kun in 1970.
