I'm wondering what was Akihito real work, he had family involved company or something else.

Makoto only had one small breakthrough, his currently busy with one-shot and is depressed it might not be noticed, after all Japan is filled with plenty people like him but his life left him with a huge baggage of inferiority complex.

Makoto should take advantage and live with Mayuko until then, this should solve his issues with place to stay, but on the other hand his work and Akihito were totally different, his true identity would be at risk, which would flip Mayuko off...

Still, it's kinda fucked up Makoto is taking advantage of mentally ill Mayuko to settle his thirst by sexual desire, instead doing what she really wants, simple relationship like when they went to the sea on a date.
