It's not fake, not a simulation, this was explained the first time the MC talked to a God. All the worlds are completely separate, but they used the closed server explanation to explain that only some higher dimensional being could have smuggled the MC from his world to the one in the story, and given his soul the bare minimum of modifications so that he would survive.
