pretty obvious one of three outcomes:

1. MC's revenge feels hollow, realizes he's just as bad as the bullies (including the girl's past self that he's mad at,) yada yada change of heart, big apology, and he goes back to protecting her. we get a somewhat-happy ending and some catharsis.
2. MC's revenge feels hollow, he feels cheated out of his revenge because the girl's not reacting the way he wants, he crashes out and tells her it was all a plot to get back at her for bullying him, maybe even saying it in front of the whole group. he gets ostracized by the class even worse for being so petty and roundabout, no longer has even the support of the girl. unhappy ending but he deserves it so we get some catharsis.
3. MC has the same crashout, escalates, kills her and himself, and we get no catharsis. just a weird and sadistic ending

edit: actually there could be a "twist" where this was all set up by the girl as a mindfuck, she's smiling because she knows he was plotting revenge but isn't feeling like he's getting it. since revenge was all that he had motivating him, and she's taken even that away, it's her total win in a sick bullying game. but i doubt it, it's hard to believe a bully would deliberately make themselves a target of the whole class just for some 5d chess to fuck with one specific guy.

• Author has done fucked up content before
• MC nearly every chapter: "Fuck this bitch, I'm going to get my vengeance on her"
• MC is so mentally fucked up from her past bullying that he at one point genuinely considers suicide a legitimate way to inflict misery on her
• R18 chapters show the female bully did some genuinely heinous shit to him that'd completely ruin most people (IRL, we've already had several high-profile cases where the victims committed suicide after the fact. Many of them teens)

Will author do some weird twist so it gets a happy/vanilla ending later? Sure, why not, but you really shouldn't be surprised if MC actually does lash out at her at some point.

I just read through the entire R-18 version. From what I can tell, he does have some love for her but at the same time genuinely hates her and thinks she's an "idiot", while the girl is already very much broken and devoted herself to him entirely, willing to suffer whatever if it means he forgives her.

This is just my guess, but she's smiling here because she thinks she deserves it; besides he's actually rougher than this when they have sex. As long as it's MC hurting her it's fine to her. I guess with that in mind a happy ending is actually far more likely than it appears, but this is not a remotely healthy relationship.

Sidenote, but it's always interesting how some people can judge if others are good or bad (and ergo if certain actions against them are good or bad) simply by seeing or not them being evil.

If that chapter was shown in the main story, a lot more people would've been understand of MC, but since it wasn't shown, but only implied, then MC's action is clearly wrong.
