The boy's uniform resembles a gakuran which is usually a middle school uniform at least from my manga knowledge. Plus the title is called Science Club girl. There is a non zero chance both are middle school students similar to the other work by the same author.

I suspect "doctor" is just her nickname due to being a mad scientist. Well, in her case, more mad than scientist.
Or it might be her name, as with Hakase Satomi from Mahou Sensei Negima. (Except that this girl is called 博士, and Satomi's name is 葉加瀬 (and Satomi's nickname is 博士 because it sounds the same and she is a genius scientist).
