Yotsuba initially partook in the bullying but regrets it Uichi killed himself. (This one is pretty obvious people cmon)
Fuu is very sus and likely the mastermind.

My 2 cents

For yotsuha it's probably that she wasn't there when it happened instead of she having anything to do directly, the new friend would be too easy and the motivation to blurry unlike with yae

it's not them either, but the mastermind is using them to hurt Uichi and the Mastermind is manipulating the sister too, not to hurt Uichi directly like with yae but to destroy the family in a chain reaction.

I don't think survivor's guilt would manifest as "I killed him too".
Her lack of action would, she was the one that gave Uichi the idea to become a vet, and she probably attributes that to some of his stress. Plus her being absent for all that time definitely played a factor, either way she blames herself for lack of action. I doubt she rejected him in earlier runs too considering how they were when they were kids, but we'll see. Personally my guess is guilt from inaction.

I'm still kinda confused about why this mastermind matters, or why Yotsuha seems to be in such a hurry. She says in chapter 16 she had already fixed Uchi's bullying when she realized there was another mastermind and then rewinded 6 whole months. Doesn't that mean the original mastermind had given up?
Hell, she didn't even have proof there was a real mastermind before she got hold of the stalker's phone this loop right?
There's a good chance Yotsuha has as much PTSD as Uchi if she rewound 6 freaking months to restart the whole process just for a possibility. This is 100% gonna end with she as broken as Uchi originally was and him helping her with romance tropes.

I wonder if the mastermind is a relative. Like the father had a child with someone else and he discarded them so they want to destroy MCs family as they blame him for taking the father away. But that would be too basic. Thanks for chapter!!!

I think, that maybe Fuu remembers.
If you consider their closeness and her status, I think she is the most likely source of the time machine, and thus could be able to travel with her into the past.

Maybe she too tried to save Uichi and failed, which is the reason for her to tell Yotsuha to accept his death.

She either could be the greatest wingwoman of all time or she could be the mastermind behind this

I wonder if the author would do the same thing twice. A Supposed friend, who likes Yotsuba and wants to get rid of the main because of it, seems rather repetitive. Also if we think about a mastermind he wouldn't help someone who wants the same girl except blaming him at the end to get her.
Also, another time traveler would make the Story pretty Paradoxical, what would be the point of the Mastermind being a time traveler if that power would amount to nothing since he wouldn't change the path Yotsuba is on but would create a different one.
So I think the best bet is a Family member or some one envious of the main.
Yeah, I'm thinking maybe a sibling? Didn't he have a sister she yelled to in the first issue?

Hrm, there's another member of the four leaf clover group nobody seems to be thinking of...

What if a time-travelling Uichi is behind it? Think of it like a grandfather paradox sort of thing - she goes back in time to save him, succeeds, then they hook up. Then he goes back in time to make sure it all happens.

Edit: That's just my theory as of the time this chapter was the latest. If the manga actually ends up that way, it's pure coincidence.
