MD only links the official chapters that have websites and aren't locked behind a key like system. I haven't seen them link apps either.

MD has quite a few rules for uploading, one of which is quality (there are still some crappy mtl on here dont get me wrong) but most stuff done by mtl groups quickly you wont find on here. Along with multiple rules for official translations to be posted and such.

Yeah you're right, then he's going to return a sword which looks nothing like the one he borrowed with a proud smile (the sword also may or may not be empowered with his and a dragon's blood)
Or, possibly, his mentor really wasn’t much better than he was at the time they parted, the mentor tried something risky in order to “prove” he was better than MC shortly after MC went into his coma, and the result was the destruction of civilization. That would be somewhat less cliché, although also already done before.
(My favorite collapse-of-magical-civilization-to-swords-and-sorcery-level explanation was in the D&D parody Nodwick, although the site crashed and the storyline, which was basically the last one posted before the crash, was lost: ancient civilization was super advanced, but this one guy was desperate to see what would be discovered in the future, and built a time machine to travel to the strip’s “present day”… and it turned out that his super-advanced half-magic half-computer combination-watch-and-PDA was explicitly not time-travel-safe, so when he set off for the future, he released a sort of magical EMP that destroyed everything, and arrived in a sort of Medieval pastiche regressed world which was explicitly his own fault. And the long-recurring villain Count Repugsive turned out to be an ancient — and female — doctor who was turned into an undead in the catastrophe, and was in fact the inventor of the magical healing duct tape used by the strip’s clerics.)
In RPGs, which all these swords-and-sorcery worlds are usually based on, they breathe anything. I’m pretty sure every single one of your examples is in one edition or another of D&D, along with other things like mud and poison, because frankly breathing fire is already ridiculously impossible enough to justify anything else instead.
