I'd assume it was a concussion considering they fell off the roof but considering MC was calling Madoka "god" and basically worshiping her , i can imagine her having better instincts or just having some mysterious sense to her

Or if this was a 'normal' toxic yuri story without the body swap, if she really did change personality/lose memories i wonder if Madoka would've tried to make /mold Sachiko into her ideal preference
