It seems like the reason Mr Bunny is shaped like a bunny and not like a keyhole is because he was able to rip out that metal thing in his head. I'm guessing that metal plate was likely how Akutsu was controlling him.
Also, the swirly text near Ms Bury and Claire suggests they aren't 100% under control either. There's just enough of themselves left in there for them to continue suffering.

yeaaa i dont think of them are under Akutsu's control, they had to drill holes in Gren's head multiple times after all

I think the end of the chapter is basically a time lapse and they all look like little cartoon frogs now, but there's a slim chance that they still look disturbing and realistic like they did on the operating table and they're just seeing everything through a cartoon filter
