I investigated it before. Kinosaki is never completely confirmed to be male in those first chapters, and tricking Gero that she's a boy in order to be able to help him better would be in her wheelhouse...

You don't get to see 'his' bare chest, but it does seem quite flat, and the long hair does seem to be a wig.
