We know cold/flu exist because there was a chapter where Xu Qing got it iirc....and Jiang He also entered his room. It was never shown if she also got it in the 2 or almost years she's been in the modern times....either she didn't thanks to her martial training/healty body or she did and Xu Qing simply went and bought medicine
The author is painting a very realistic modern world while the "past" in this story is pure fantasy. Ancient people were not stronger than modern people and were not more cruel and violent.
