Getting a ragebait vibe from this but just in case you're serious, I'm pretty confident this was a flashback chapter and he just doesn't yet realize that it was Nanjou-san in the flashback. It's a typical "he was always in love with her and just didn't know it" kinda scenario but to your point, when she changed her look and demeanor, he clearly doesn't recognize her.

Honestly it’s kinda feels like this manga ran out of steam after the firework when they’ve done it the first time. But I guess bringing up the past now maybe this will finally finishing some loose ends of why she decided to do what she did and how they gonna overcome it. I the ending might not be far off. Good thing too because the artist can solely focus on another title which I really looking forward to.
