He might be a 3rd party!! It seems like the organization was part of the "side business" since he didn't tell the yakuza about them. I hope they talk more about where this doctor came from too

well there was a previous? boss that died before he took over , so i imagine it's not something they would've approved of maybe but considering midori and nezumi were trained as killers too i would've expected it to be normal unless it was shut down and they were literally the only two girls who survived , since yakuza has some 'honor code' and 'sworn brothers' so even maybe they couldn't kill him for this or so/by the time midori and nezumi were taken, he was too high up for anyone else to fight back

tho given his size i imagine most wouldn't have defied him anyways even if they didn't approve

Seems I was right on the money, the child trafficking was a separate organization from the yakuza and Butama was secretely a part of both.
Don't go in there, Midori. That yakuza goon was a lifesaver.

This is just a guess but he might be the grown up version of the kid shown in chapter 49?
