Let me guess, grandma will have complications before/during/after the surgery, whether she dies immediately or not this will cause her the inability to continue and that's when MC will give her the motivation to finish, it will be a moving moment and one of them might even confess, but no real progress with their relationship.

I really like the art, but in terms of righting it is very formulaic.
