In games like those, most robes and armor come with their own underwear. (the exception being bikini armors - many of them make it clear there's nothing underneath them)
Unless the player has been using a lewd mod to change it.

I'm just confused because she had normal clothing on, requested to equip underwear, and now she's only wearing the sexy underwear. I guess it's just RPG logic throwing me through a loop

Basically, both her usual robes and the sexy underwear go into "armor" slot. And there is no separate "underwear" slot, so they have to include underwear (unless they are underwear).
Some games allow one to remove armor entirely, and that causes underwear to appear, but any armor overrides underwear.
