Going back to chapter 41 he said his:
1. girlfriend in middle school (unnamed afaik);
2. ex-girlfriend Yuu (the one with exploding clothes);
3. ??? (Hinted at being watanabe); and
4. Akari (the milf)

So this means that little bit at the end where it strongly suggested that #3 was the guy on his school trip was actually a fake out, and it was his sister instead.
