Drugs can be super weird, even ones that have less fantastical effects than transforming your entire body overnight. It's possible that at some sort of critical mass the effect just became permanent instantly, if Mihari was steadily scaling up the dosage to counteract Mahiro developing a tolerance, or just increasing body mass. Or maybe some sort of thing where once Mahiro's female form passed puberty the effect changed to become permanent.

since it did change mahiro's form overnight, it probably just needs a certain amount in a given time to completely overwrite DNA(???), since like, that would be the body's instruction manual or something??
though i dont really know, i didnt study these stuff much... biology might actually be a pretty good subject to learn for this...
that said, i have many areas where i dont know how they work, basically, at what point does DNA affect the body? if there is enough body mass, wouldn't cell replacement just continue as such? it might be possible that mihari's lab had already stumbled on the cure to cancer...

it may be possible that returning back to a younger age was an actual necessity for the drug to work properly, back to pre-pubescent where the body hasnt developed most of the distingishing external factors of either male or female sexes from naturally produced hormones...
in our current medical practices, hormones are commonly administered to people who want to transition or develop the traits of the opposite sex, but it commonly works better to those who hadnt already been exposed to puberty... is it possible to just fool the body to produce the desired hormones instead of the original ones? in such case, it would mean a permanence after transition... (this assumption was made in the expectation of the reproductive organs being capable of being switched, impossible with current known practices, im speaking like a madman)

That's actually a good point I didn't think about on the main story. The age regression to middle schooler kinda makes more sense now with the logic you laid out with it being a requirement for the TS drug to work than a side effect.

I would say that it would be better to probably study animals that have the ability to swap between sexes rather than give drugs to kids before they finish developing the part of their brain that deals with reasoning and stuff.

Teenagers and children do stupid stuff all the time… I don’t think it’s a good idea to support them do something that would affect them for the rest of their lives. After they become adults do what ever they want they are responsible for themselves at that point
