She should get breast reduction surgery because she is going to have serious back problems. Those probably weigh 20 kg each. Imagine having to live your life with 40 kg of weight strapped to your chest.

It seems pretty obvious at this point that she's dealing with it because she thinks it appeals to him (which it does, and his protestations to the contrary are clearly not fooling anyone). I suspect where this goes and what she decides on is intrinsically linked to him, with two clear end states being "they work together and find a way for her to comfortably get back into swimming, which gets her to love herself more as she is" and "he makes it clear he'd be happier if she wasn't that huge, which gets her to do something about it".

That being said, she's what, 14-15? I don't know Japanese medical law for minors, but that may also be a non-trivial factor.
