Too lazy to check the RAW but by context I'm 90% sure it's TL being too innocent to realize the iku(going)/kuru(coming) are referring to sexual climax.

i.e. a fake sadist will push to climax, but a real sadist will deny it

I mean, to me, the difference is that a fake sadist will just mock you and try to tear you down, while a real one will give you a false sense of understanding. They'll encourage you, make sure you don’t give up all while savoring every second of your struggle. (Think Vitra from FGO, for those who know.) ... Ok I stop before I out myself too munch.
