The character in the cover is from an artwork drawn by an artist named Whooosaku, but curiously they don't seem to be involved in this. And I can't find any actual information about this... doujinshi.

Someone didn't just slap some leaves and text on that artwork and call it a day, did they?

Yep, stolen art modified and used as a cover (by the scan groups, most likely because it's in their releases).

Shize's Pixiv does not have this cover or use this art. The full release in certain places also do not use Whooosaku's art as a cover.
