It's definitely more of a priority thing. Karin knows and understands how much they care about each other that comes with their bond as siblings (and heck twins) but at the same time, she's worried that he'll always make Akane his first priority and not care enough for her. Which she accepts but she also is in a dilemma about. And also their relationship not changing despite their status adds to that
