I don't think the extra is supposed to have a joke per se. It's a what if they were opposite genders kinda thing. Maybe the joke is that nothing really changed? Sacchan is still a hornball and Ken-chan is still submissive.

Alright I know sarcasm is hard to get through text but the fact that you all understand the premise of this story, means at least someone should have picked up on it. "Feminine male, masculine female," and the third volume has homeboy in cat girl maid costume, them actual being gender bent is a bit redundant and funny. Homeboy might as well just be wearing a wig.
