One of the worst, if not the worst curse word(really a phrase tho) in the Japanese language. Partly due to their culture of conformity and partly due to their less-than-friendly approach to mental health as a society. It's something you normally wouldn't even hear in fiction outside of delinquent manga or one with a lot of scumbags (think marauders in fantasy settings).

In fact, simply implying someone feels different or there's something wrong with their head is considered rude over there.
