well, last chapter it seems she 'created' it somehow? unless it somehow leaked/combined with some existing afterlife/purgatory or so but i think the angel at the beginning did say something about 'if you can't figure out if you're dead or in a coma you'll die for real/be taken away' or something like that

Rather than her forgetting, it's definitely possible that the Nii she saw by the lighthouse was a totally distinct entity. It's possible she was a twin, for example, that just happened to die similarly, but it's also possible there's just more than one instance of her "ghost."
