It showed sis slumped on the desk viewed from behind her and itsuki noticed the stone in the bottle that used to contain the message itsuki wrote. So either this is fake or it's actually how his sis looks.

Wait... I'm probably mixing mangas up, but I think his sister already showed up, in a previous chapter where he managed to send a letter to his sister on Earth. And the girl shown it's not her.

She's shown in a flashback on the first page of chapter 44. She looks the same as she does here only with slightly less messy hair.
