Sleep deprivation pretty much has the same effects as inebriation. It's completely possible this guy sleepwalks through his days so blasted off 4-6 hours sleep he doesn't notice very obvious things.

I would imagine Yamada puts up cutesy voice for customers' hospitality sake while 'Tayama' speaks in her natural voice, or maybe even lowers it a bit.
And as mentioned above, combined with speep depravation (and general block-headness lol) this may be enough for him not to notice.
I just...hope mangaka either will keep this part interesting or invents something new, cause it slowly starts to get old for me.

The stupid thing is that she isn't even trying to hide her identity.
I'm guessing that this is one of those cases where you've made the mistake once and your mind just anchors onto it without even considering alternatives. So it would take far more evidence thrown in his face to understand they're the same person than it would if he never assumed they weren't in the first place.

I haven't read the raws but I just realised probably, Tayama is just Yamada with the last character (if it's 田, normally it's pronounced ta but it makes the da sound when put after some words) put in front of the name.

It's not very subtle and I think Sasaki is just too tired and also biased to notice all the tells.

He's stressed all the time. This causes anxiety and that's why people smoke amongst other things. Also in his mind Tayama is some kind of wild girl while Yamada is the opposite. This is why even if his eyes could see the similarities, his mind will focus on the differences.
