Since this dragon seems to be a shapeshifter, maybe Damian doesn't just have the hear of the dragon but IS the dragon.
Along with his guilt, and what the spirit said about a possible good dragon before. I'll guess either he has multiple personalities or he goes on uncontrolled rampages when his power erupts. Making him feel guilty even though he doesn't have any direct control over it.
