Shiori likes Rei, but got rejected.
Now she's using Sonoe as a rebound.
Sonoe likes Ichika, but can't make any progress because she's fooling around with Shiori.
Maybe she thinks she can pick her up while keeping her mistress.
Ichika likes Sonoe, but all the drama has her confused.
Now Rei is using Ichika either to get rid of Shiori, or maybe to mess with Sonoe - or both.
Maybe she's hoping to give her indecisive sister a push. Or testing everyone so they can all figure out what they want. Or just causing trouble for her own amusement.

All 3 of them are too much drama. Ichika should do herself a favor and stay away from all of them.

So yeah, Shiori settled for younger sister, even though she likes the elder one. I prescribe some prophylactic boot-to-the-head for Rei, so that she would stop being a bitch and accept poor Shiori. Then our main heroine can have Sonoe all to herself. I believe, she is the one who should give out the medicine
