Well, even still, Marisa was born and raised in the human village and frequents there, so in the eyes of the villagers it’d be the same. The reason why humans from the village becoming youkai is forbidden is likely because the idea is contagious. Gensokyo is built so the youkai can feed off the fear and belief from the villagers, so to that end the human villagers has to remain human.
