In the first page we can see how careless Haruka is, she nearly hit by vehicle but got saved by him

And then when she going home after had sex with him, she probably careless again and right now she actually got hit by vehicle because he's not by her side to save her.

IN CONCLUSION:
Bad ending = She died
Good ending = She just got injured
Ultimate ending = She injured exactly like him, and then he'll rape her to get her memory back just like she did to him earlier
